Early clinical signs of endocrine ophthalmopathy in different function of the thyroid

Endocrine ophthamopathy (EOP) develops in the persons of working age (30—50 years old), not infrequently leading to the sable invalidization of the patient.

Therefore early diagnostics and timely treatment of patients with EOP has the important social significance.

Purpose: to study early clinical symptoms of EOP depending on the function of the thyroid.

Material and methods. There were studied 332 case histories of patients with EOP that were treated in SI «The institute of eye diseases and tissue named after V. P. Filatov of MA of Ukraine» in 2002—2013. There was created the electronic database containing information of the patient, anamnesis of the disease, data of the clinical and instrument examination of patients. Statistical processing of the data was conducted with the aid of the program «Statistics 9».

Results. 0/332patients there were 217(65.36 %) women and 115 (34.64 %) men. The age of the patients ranged from 18 to 83, the mean age was 50.5 (SD13.4). EOP is encountered more frequently in the persons capable of working (75.0—92.5 %) and 1.9 times more often in women. EOP developed against the background of hyperthyroidism (64.46 %), hypothyroidism (21.69 %) and euthyroidism (13.85 %). There were 179 (53.92 %) urban inhabitants, village — 153patients (46.08 %). Reliable difference in the character of the disturbance of the thyroid function of patients with EOP between them was not revealed (%2 =0.17; p = 0.92). Bilateral exophthalmia is encountered more frequently than unilateral (72.9 % and 27.1 % respectively). Right and left orbits were affected equally frequently independent of the thyroid function and sex of the patient.

The manifestation of clinical symptoms of EOP with bilateral exophthalmia can be asymmetric.

Reliable difference in the occurrence rate of the basic clinical signs of EOP in different thyroid function is not revealed. The first manifestation of EOP was protrusion of the eyeball (84.94 %), which was combined with edema of the eyelids in 34.34 % of cases, diplopia — in 31.62 %, epiphora — in 10.84 %, reduction in the visual acuity — in 1.81 %, ptosis — in 1.20 %.

Conclusions. EOP develops more frequently in the persons of working age (75.0— 92.5 %) against the background of hyperthyroidism (64.46 %) and 1.9 times more often in women. The first manifestation of EOP independent of the thyroid function is protrusion of the eyeball (84.94 %). There is no reliable in the frequency of basicclinical signs of EOP in different thyroid function.
